Undoubtedly everything is executed to a generally excellent level. But there are a couple of issues:
There is a jarring flash cut when Nina first sees the imaginary friend. It may be intentional on your part but there are a number of other effects happening simultaneously which mean that it stands out.
The ancillary piece carries no image of Nina anywhere on it. Impressively rendered though this is, it feels like an oversight to me. The cracks etc work well, but surely it is Nina's character who is cracking up. Is there anyway to involve her in the shot, maybe a shadow image, him on her?
I think it is a bit of an issue, the lack of dialogue or any exposition through dialogue. However it is a bit late to do anything about it now and I think the general idea does come across. On a technical level, you still do enough well here to get a high mark.
However, if you were inclined to have more of a fiddle, you could bookend the film with the interview room footage. If there is even a short clip of that at the start then the audience immediately knows that something bad has happened and they will be looking for the clues as a result.
As for the end credits - Job title smaller than persons name, as a rule.
